The number on the toilet paper is not a 3 !!! It's cut into half.
Also a nice trick with the circle number (two different ones).

I am almost out but can't seem to get the math bit. I thought I knew the order of operations but it's not working.

does the half star mean that the whole star is 6???

I think the star is an 8. Visualize it mirrored.

Hmm, pretty sure I've done everything above already but can't get correct number. Have 2 x circle numbers and recognised the star is in half (8?) and have multiplied them before adding final number. What have I missed?

smarties : circle from stethoscope 7, circle from clock 9, star 8 , square 4

Clock * stethoscope * star + square=

9*7*8+4 = 504+4 = 508
